Research Purpose:

To study the effects on blood pressure of different levels of sodium restriction, in conjunction with the DASH diet, in persons with and without hypertension.

Inclusion Criteria:

1. >=22 years of age

2. Average systolic pressure:120-159 mm Hg

3. Average diastolic pressure: 80-95 mm Hg

4. Targeted 50% black and 50% women

Exclusion Criteria:

1. heart disease

2. renal insufficiency

3. poorly controlled hyperlipidemia or diabetes mellitus

6. >14 alcoholic drinks/wk

7. use of antihypertensive drugs

Description of Study Protocol:

Recruitment: 4 clinical centers.  Recruitment was done to target an enrollment that was 50% blacks and 50% women

Design:  2 week run-in period, in which subjects ate a high-sodium control diet and then were randomly assigned to follow either the control (usual American diet) or DASH diet according to a parallel-group design.

Blinding Used:  The participants and the and the dietary staff were unaware of the outcome data.  The personnel collecting the outcome data were unaware of the participants' dietary assignments.

Intervention:

During the intervention period they ate diets at each of 3 levels of sodium in random order with  a crossover design for 30 days.  Each participant's energy intake was adjusted to assure constant weight during study.  All of participant's food was provided through the study.  The daily sodium intake was proporotional to the individual participants' energy requirements.  The three sodium levels were:

  •  150 mmol/day or 3.5 g of sodium
  •  100 mmol/day or 2.3 g of sodium
  •  50 mmol/day 

The daily sodium intake wask proportionate to the total energy requirements of each individual, so that larger or very active persons would receive more food, and therefore more sodium, than smaller or less active persons.

For each sodium level, the subjects were assigned to either a control diet or a DASH diet.  The DASH diet emphasizes fruits, vegetables, low-fat dairy foods, and includes nuts, poultry and fish; and contains smaller amounts of red meats and sweetened beverages than the typical US diet.

Statistical Analysis

  • the planned sample size of 400 was calculated in order to provide the study with a ower of 90 percent to detect a difference in systolic blood pressure of 2.1 mm HG between sodium levels, and a difference of 3.0 mm Hg between the DASH and control diets at each sodium level.

Summary of Results:

Other Findings

As compared with the control diet with a high sodium level, the DASH diet with a low sodium level led to a mean systolic blood pressure that was 7.1 mm Hg lower in subjects without hypertension and 11.5 mm Hg lower in subjects with hypertension.

Reducing sodium intake from 3350 mg to 2300 mg/d decreased systolic blood pressure by 2.1 mm Hg (P<0.001) during the control diet and by 1.3 mm Hg (P<0.03) during the DASH diet.

Reducing sodium intake from 2300 mg to 1150 mg/d caused additional decrease of 4.6 mm Hg during the control diet (P<0.001) and 1.7 mm Hg during the DASH diet (P<0.01).

The effects of sodium were observed in subjects with and without hypertension, blacks and those of other races, and women and men.

Author Conclusion:

The reduction of sodium intake to levels below the current recommendation of 100 mmol (3,350 mg) sodium/d and the DASH diet both decrease blood pressure substantially, with greater effects in combination than singly. Long-term health benefits will depend on the ability of people to make long-lasting dietary changes and the increased availability of lower sodium foods.

This study was strictly an evaluation of the biological effects of reducing sodium on the usual or DASH diet.

This study did not include persons with diabetes and should be repeated in individuals with diabetes.

The DASH diet lowers blood pressure and adds a variety of foods to the diet and is a more positive approach than avoiding sodium.
