To develop a new equation to predict resting energy needs from data collected in heart failure (HF) patients that takes into account easily measured clinical variables and symptom severity. 
Inclusion Criteria:
  • Symptomatic HF during ordinary physical activities
  • Mean left ventricular ejection fraction, documented by radionuclide ventriculography of under 0.38
  • New York Heart Association Class III or IV
  • Weight stable for at least two months.

    • Resting energy expenditure (REE) was measured for 45 minutes the morning after a 12-hour overnight fast in a darkened and temperature controlled room
    • Oxygen consumption and carbon dioxide production were continuously measured by indirect calorimetry, using the ventilated-hood technique (Deltratrac)
    • Oxygen consumption and carbon dioxide production were converted to a caloric equivalent, using the Weir equation. 
  • Variable Two: Fat-free mass and fat mass were measured using a total body scan with dual-energy X-ray absorptiometry (Lunar DPX-L)
  • Peak VO2 was assessed using an open circuit gas analysis system (Sensormedics) during a maximal treadmill exercise test to exhaustion.

Other Findings

  • REE was strongly correlated with age (R=-0.46), body weight (R=0.74), fasting glucose (R=0.69), peak VO2 (R=0.58) and NYHA classification of HF (R=0.30); all at P<0.05
  • The independent predictors of REE were body weight (R2=55%), fasting glucose (R2=19%), plasma albumin (R2=6%) and NYHA class (R2=3%). These variables predicted REE with a root mean square error of 130kcal per day (total R2=83%). The regression equation developed from these predictors is REE (kcal per day)=12.2 (weight in kg)+1.6 (glucose in g per dL)+103 (NYHA; III, IV)-144 (albumin in mg per dL)+755. 

 

 

Author Conclusion:

We have provided a new equation to predict resting energy needs in heart failure patients, based upon simple, readily available measurements including symptom severity.

  • Two of the predictors used in the equation for predicting REE, glucose and albumin, were not mentioned in the methodology description. The equation only applies to HF patients with NYHA Class III and IV. 
  • Poehlman ET was charged with falsifying data on some of his studies. The Office of Research Integrity listed the studies that were affected by this misconduct. This paper was not one of those; therefore it is being considered and reviewed.
