Vitamin E
To explore the effect of vitamin E supplementation on the develoment of congestive heart failure (CHF) in patients enrolled in the GISSI-Prevenzione trial.
- Enrolled in the GISSI-Prevenzione trial
- No clinical diagnosis of heart failure at time of enrollment
- Echocardiographic measurement of ejection fraction at baseline.
- Clinical diagnosis of heart failure
- Use of antiotensin-converting enzyme inhibitors, β-blockers, diuretics or nitrates for CHF management.
Design
Open label, randomized trial.
Blinding Used
Ad hoc committee determining development of CHF or death from CHF blinded to treatment assignment.
Intervention
- GISSI-Prevenzione study enrolled 11,323 patients with recent (less than three months) myocardial infarction (MI)
- Subjects randomized to one of the following interventions:
- 1.0g per day n-3 polyunsaturated fatty acids
- 300mg per day vitamin E
- Both supplements
- Control (neither supplement)
- 3.5-year follow-up (efficacy for n-3 polyunsaturated fatty acids).
Statistical Analysis
- Student's T-test for continuous variables or X2 test for categorical variables
- Multivariate Cox proportional hazards model adjusted for age, gender, smoking habits, diabetes, arterial hypertension, peripheral vascular disease, ejection fraction, residual ischemia, electrical instability and n-3 polyunsaturated fatty acids
- Cox proportional hazards event-free survival curves adjusted for covariates were plotted using the corrected group prognosis method.

Baseline: Echocardiographic measurement of ejection fraction.

- Left ventricular dysfunction (measured by ejection fraction).

- There were no differences between the groups for the following characteristics: Arterial hypertension, diabetes mellitus, smoking status, claudication, previous MI, total cholesterol, New York Heart Association functional class II, ejection fraction, electrical instability, residual ischemia, use of antiplatelet drugs, β-blockers or angiotensin-converting enzyme inhibitors.

New Cases of Heart Failure According to Tertiles of Ejection Fraction and Vitamin E Assignment
- During 3.5 years of follow-up, 2.6% of patients required hospitalization for or died from HF
- Patients assigned to the vitamin E group had a statistically nonsignificant increased risk of developing HF (95% CI: 0.92 to 1.56, P=0.18)
- Patients with left ventricular dysfunction (ejection fraction less than 50%, mean 43%, range 19% to 49%) who were assigned to the vitamin E group had a statistically significant 50% increase in risk of developing HF (P=0.0345)
- Analysis for interaction between experimental treatments, n-3 polyunsaturated fatty acids and vitamin E were not statistically significant (P>0.05).
In individuals with a previous history of MI and who have mild to moderate left ventricular dysfunction, vitamin E may be harmful.
