To evaluate the dietary habits of adults with longstanding celiac disease.

Inclusion Criteria:
  • All patients had diagnosis confirmed by jejunal biopsy showing lesions compatible with celiac disease
  • Subjects were started on GFD and showed signs of remission within 1 year
Exclusion Criteria:
  • 8 subjects excluded for after showing persistent intestinal lesions due to non-compliance with GFD
  • Celiac patients with concomitant dermatitis herpetiformis were not enrolled
Description of Study Protocol:

Recruitment

Study was performed in 1996-1997, subjects were recruited from a cohort of middle-aged celiac patients who had been on a strict GFD for 8 -12 years.  49 patients were recruited from the 65 living adult celiac patients aged 45 - 64 years diagnosed in routine practice at 6 gastroenterology departments in southeast Sweden in 1984-1988.  Controls consisted of a general population sample of the same age.

Statistical Analysis

Results were tested using t test.  To avoid associations occurring by chance an association was considered significant if true for both men and women or true for at least for either of them with the other showing an association in the same direction.

Other Findings

In the celiac patients, routine blood lab work was within normal ranges.

The celiac patients showed the same energy intake and relative contributions of protein, fat and carbohydrate as controls, but had a lower intake of fibre, niacin equivalents, folate, vitamin B12, calcium, phosphorus and zinc (all P < 0.05).

In relation to recommended intakes, low levels of fibres, folate, vitamin E and selenium were seen in both celiacs and controls.

Significant differences between celiac patients and controls were seen for half of the food groups under study and this was particularly true for women. 

Author Conclusion:
In conclusion, the gluten-free diet taken by Swedish adult celiac patients appears to be nutritionally unbalanced and in need of improvement in certain respects.  To this end, we firmly believe that regularly visiting a dietitian should be an urgent component of the surveillance of celiac disease, as it is in the care of diabetes patients.  One important step to ensure a more adequate diet would be to encourage celiac patients to introduce oat-based options into the diet, as well as increase their intake of fresh fruits and greens.
