Research Purpose:
- To establish the relationship between nut consumption and risk of coronary heart disease (CHD) in a cohort of women from the Nurses' Health Study.
Inclusion Criteria:
- Women in the Nurses' Health Study completing 1980 dietary questionnaire.
Exclusion Criteria:
- Missing data on questionnaires (questionnaires with 10 or more blank items)
- Questionnaires with implausibly high or low scores for energy intakes or for total food intake
- Women with previously diagnosed cancer, angina, myocardial infarction (MI), stroke or other CHD diseases.
Description of Study Protocol:
- Nurses' Health Study collected baseline-detailed information on medical history and lifestyle of 121,700 registered female nurses 30 to 55 years of age. Every two years, the women received a follow-up questionnaire to identify new potential risk factors for diseases as well as newly diagnosed diseases such as CHD.
- 1980 dietary questionnaire contained 61 items; expanded to 116 item dietary questionnaire in 1984, 1986 and 1990.
- 1980 and 1984 dietary questionnaires asked how often during the previous year they had consumed a unit (1 oz) of nuts.
- 1986 and 1990 dietary questionnaires asked if peanuts or other nuts were consumed.
- Consumption of peanut butter (1 TB = 1 oz of nuts) was assessed in 1980, 84, 86 and 90.
Data Collection Summary:
- Outcome measures: Major coronary heart disease including non-fatal MI and fatal CHD.
Description of Actual Data Sample:
- 1980 baseline: 86,016 women, 34 to 59 years of age.
Summary of Results:
- 14 years of follow-up 1255 major coronary disease events (861 cases of non-fatal MI and 394 cases of fatal CHD)
- Follow-up rate for non-fatal coronary disease events was 98% of the total potential person years of follow-up
- Follow up for deaths was 98% complete.
- After adjustment for age, smoking and other known risk factors for CHD, women who ate >5 units (5 oz) of nuts/week had a significantly decreased risk of total CHD RR 0.65, 95% CI, 0.47 to 0.89, P for trend=0.0009) compared to women who never ate nuts or who ate <1 oz/month.
- Frequent nut consumption was associated with a ¯ risk of fatal CHD and non-fatal MI.
- These data support a role for nuts in reducing the risk of CHD. In addition to the beneficial effects on blood lipids, nuts may protect against CHD through other mechanisms: As a source of arginine (the precursor of nitric oxide, an endothelium derived relaxing factor), which is a potent vasodilator and can inhibit platelet adhesion and aggregation; walnuts are high in linolenic acid, associated with reduced risk of CHD; or other benefits—high amounts of magnesium, copper, folic acid, protein, potassium, fiber and vitamin E.

- When separated, both consumption of peanuts and other nuts were inversely associated with risk of CHD. Peanut butter was only weakly associated with risk.
Author Conclusion:
- FFQ validated. Frequent nut consumption was associated with decreased intakes of trans fatty acids, and increased intakes of PUFA and fiber, more multivitamin and vitamin E use, more exercise.
- Frequent nut consumption was associated with a reduced risk of both fatal CHD and non-fatal myocardial infarction.
- Author concluded that nuts can be included as a part of a healthy diet and that these data support a role for nuts in reducing coronary heart disease risk.
