Research Purpose:

To identify the prevalence of overweight and obesity in 2000 students and its external modifiable causes in order to implement efficient integrated community based interventions aimed at preventing and controlling overweight and obesity from early life.

Inclusion Criteria:

Students aged 11 to 18 years from randomly sampled guidance and high schools in different urban and rural areas. Also included was the students’ parents (n=2000) and school staff (n=500).

Description of Study Protocol:

Recruitment

Randomly sampled students, parents and school staff in guidance and high schools in different urban and rural areas.

Design

Collected demographic data (age and birth date, family income), anthropometric measurements (height, weight), blood pressure, HDL, LDL and triglyceride levels, three 24 hour food records and a FFQ, parent’s questionnaire (behavior, attitude, skills and knowledge).

Statistical Analysis

T test (compare the mean of variables), Yates corrected chi-square (determine differences in physical activity behaviors and frequency of food consumption), Pearson’s correlation test (determine the relationship between different BMI percentiles and the mean serum lipid and lipoprotein levels and blood pressures), multiple linear regression (associations between BMI and eating and exercise behaviors).

Description of Actual Data Sample:

Initial N: 2000

Attrition (final N): 2000 (1000 girls, 1000 boys) plus students’ parents (n=2000) and school staff (n=500)

Age: 11 to 18 years

Ethnicity: Iranian

Anthropometrics: Higher prevalence of overweight and obesity in girls than in boys (10.7±1.1 and 2.9±0.1% versus 7.4±0.9 and 1.9±0.1%, respectively, p<0.05). Mean BMI value was significantly higher in urban than rural areas (25.4±5.2 versus 23.2±7.1, p<0.05).

Summary of Results:

Physical Activity and Television Viewing

There was a significant difference with regard to regular physical activity according to:

  • the year at school
  • participation in team sports between girls and boys 
  • BMI lower or higher than the 85th percentile for sex

The time spent watching television was significantly greater for subjects with BMI>85th percentile than for other subjects (300±20 versus 240±30 minutes/day, p<0.05).

Diet

Fat (g) and percent energy from fat were not analyzed. Only food frequency was analyzed.

Between subjects with BMI lower and higher than the 85th percentile there were significant differences between the frequency of consumption of:

  • rice
  • bread
  • pasta
  • fast foods
  • high fat and salty snacks
  • fruits and vegetables

A significant linear association was shown between BMI and the frequency of consumption of rise, bread/pasta, fat/salty snacks (ß=0.06, p=0.03) and fast foods (ß=0.05, p=0.02); this associations was inverse regarding fruits and vegetables and also regarding regular physical activity performed throughout the year and not only during summer holidays.

Total energy intake did not differ between overweight or obese and non-obese subjects, but the percentage of energy derived from carbohydrates was significantly higher in the former group compared with the latter (69.4% versus 63.2%, p<0.05).

Anthropometrics

A significant correlation was shown between BMI percentiles and serum triglyceride, HDL, and systolic blood pressure (Pearson’s r=0.38, -0.32, and 0.47).

SES and Family

The prevalence of overweight and obesity in children of families with a moderate income was significantly higher than for children in families with a high income (9.3±1.7 versus 7.2±1.4%, p<0.05) and in those with lower-educated mothers (9.2±2.1 versus 11.5±2.4 years of mothers education, p<0.05).

Only 35.2% of parents and 24.7% of school staff tried to give information to obese adolescents or to change their lifestyle regarding nutrition and physical activity.

Mothers of overweight or obese adolescents preferred an overweight child and adolescent and agreed with the belief that these children would outgrow their obesity and overweight more often than mothers of adolescents with a normal weight (21.8% versus 15.2%, p<0.05).

Author Conclusion:

It can be suggested that the environment can have a decisive positive or negative impact on obesity in early life by introducing and reinforcing good or poor food and physical activity habits.
