Research Purpose:
To compare glucose profiles from home meters vs. continuous sensors.  This paper is the analysis of baseline data from a randomized clinical trial conducted by the Diabetes Research in Children Network (DirectNet).
Inclusion Criteria:

  • Type 1 diabetes with use of insulin for at least 1 year
  • Screening A1C of 7.0-11.0%

Statistical Analysis

  • Mean glucose value was calculated for each subject based on their 8-point testing measurements.
  • Pre and postprandial mean was calculated for each subject.
  • Percentage of tests within target range from both systems (8-point testing and CGMS) was calculated for each subject (target 61 - 180 mg/dl).
  • Overall CGMS mean was calculated after calculated the mean for each hour.
  • Mean concurrent CGMS values were compared with overall CGMS values using the paired t test.
  • Differences between pre and postprandial measurements (postmeal excursions) were compared from both systems.
  • Overall glycemic variability was evaluated from the CGMS measurements by use of the standard deviation and mean amplitude of glycemic excursions.
  • Mean glucose values from both systems was compared to the A1C results using Spearman correlations; slopes were calculated by least squares regression.
  • Comparisons between 8-point testing and CGMS was done using paired t test based on the paired differences from each subject.

Timing of Measurements

  • Three days of blood glucose measurements using the One Touch UltraSmart meter and test strips with measurements before each meal, 2 hours after each meal,  before bedtime, and nightime between midnight and 4 am.  Total of 8 measurements each day were to be collected by each subject.
  • Three days of measurements from the GlucoWatch G2 Biographer Continuous Glucose Monitoring System (CGMS) was requested of each subject.  Glucose measurements from the CGMS provided values every 5 minutes. Subjects were to repeat the CGMS if there were not at least 2 days of measurements collected.

Other Findings

  • Percentages of measurements within target, above target, and below target were similar between both systems (8-point testing and CGMS) and varied as expected with increasing A1C levels, except for the percentages below target levels (Rising A1C levels did not mean less hypoglycemia events in this population).
  • The CGMS was able to detect more of the nocturnal hypoglycemia.
  •  Each 1% change in A1C levels was associated with a 23 mg/dl change in overall mean by the eight-point testing and a 19 mg/dl change by the CGMS testing.

 

 

Author Conclusion:
Both methods gave similar mean glucose profiles and associations with A1C.  Advantages of the CGMS were higher density of data and better detection of postprandial peaks.  However, the CGMS may have overestimated the frequency of nocturnal lows.
