NSUP: Vitamin B12 (2008)
To examine the relationship between serum cobalamin and cognitive function in adults age 60 years and older with cobalamin deficiency.
- 60 years or older
- Serum cobalamin level less than 120pmol per L.
- Anemia (Hg less than nine grams per dL)
- Unstable medical condition
- Severe confusion
- Communication problems
- Signs of subacute combined degeneration of cord.
- Recruitment: Hong Kong residents, including free-living patients from hospital outpatient clinics or wards and homes for the aged
- Design: Randomized control trial

Intervention
- Subjects were randomly assigned to supplement and control groups
- The treatment regimen included intramuscular dose of cyanocobalamin three times in the first week, one dose per week thereafter for three weeks, followed by one monthly dose
- The supplement group's cyanocobalamin dose was one mg
- The control group received no intervention and were advised not to take any oral vitamin supplements.

The following measurements were taken at zero, three and six months:
- Serum cobalamin
- Folate
- Methylmalonic acid
- Complete blood cell count.
Dependent Variable
intramuscular dose of cobalamin.
Independent Variables
Cognitive function, as determined by Mini-Mental State Examination, digit span, similarities and block design subtests of the Wechsler Adult Intelligence Scale and Motor Function Scale of the Adult Luria-Nebraska Neuropsychological Battery.
Control Variable
Intramuscular dose of cobalamin.

The majority of subjects (25 control subjects and 17 treatment subjects) were vegetarians.
- No benefit in cognitive function from cobalamin supplementation in older people with cobalamin deficiency
- The only parameter that improved in the supplementation group was performance IQ, which also improved in the control group
- No statistical significance was found
- The study group deteriorated significantly in motor scores for kinesthesia, fine motor and oral motor (P<0.05).
- This preliminary study did not support the hypothesis that cobalamin deficiency invariably causes cognitive impairment in older people
- There remains the possiblity that cobalamin deficiency causes reversible cognitive impairment in some older people or exacerbates cognitive decline in older people with coexisiting dementing illnesses.
