Research Purpose:

The purpose of this study was to evaluate the safety and efficacy of laparoscopic adjustable silicone gastric banding 18 months after surgery.

Inclusion Criteria:

All patients who were scheduled for a laparoscopic silicone gastric banding surgery were allowed to participate in this study.

Exclusion Criteria:

There were no exclusion criteria.

Description of Study Protocol:

Design

Between November, 1996 and August, 1998, all referred patients were scheduled to receive a laparoscopic adjustable silicone gastric banding surgery. Patients were followed for 18 months following surgery to monitor intra-operative or post-surgery complications, as well as band placement and weight loss.

Data Collection Summary:

Timing of Measurements

Patients were measured for body mass index (BMI) prior to surgery. Following laparoscopic adjustable silicone gastric banding, patients were evaluated immediately following surgery for complications, up to 13 months following surgery for complications and at months one, three, six, 12 and 18 for BMI measurements.

Dependent Variables

  • Intra-operative complications immediately following surgery (three patients had bleeding from a liver injury; one patient had pneumothorax; one patient had stomach injury)
  • Additional procedures immediately following surgery (14 patients received a cholecystectomy; five patients received hiatal hernia repair; two patients received umbilical hernia repair)
  • Re-operation within three days after surgery (one patient had a bleeding trocar site; five patients required band repositioning due to outlet obstruction)
  • Complications up to 13 months following surgery (four patients had major complications; 12 patients experienced minor complications)
  • Band dislodgement following surgery (16 patients had band dislodgement and all underwent a laparoscopic procedures for band removal or displacement)
  • Weight loss 18 months after surgery (all 11 patients who underwent band repositioning continued to lose weight).

Independent Variables

A standardized laparoscopic adjustable silicone gastric banding was performed on all patients.

Control Variables

Patients were allowed to drink fluids six to 12 hours following surgery. Patients were discharged when they were able to drink fluids by mouth. All patients were followed up in the outpatient clinic.

Author Conclusion:

The author concludes that when laparoscopic adjustable silicone gastric banding is performed correctly, it is most likely the safest bariatric surgery and that it is effective with good, sustained weight loss. However, the author also cites that much longer follow-up is needed before final recommendations and conclusions can be made.
