Research Purpose:

To study the effect of dietary potassium on blood pressure in salt-sensitive and salt-resistant black adolescents.

Inclusion Criteria:
  • Healthy black adolescents (aged 13 to 16 years)
  • Normotensive
  • Within 25% of their ideal weight for their height
  • Two blood pressure measurements were within 5mm Hg or more of each other.
Exclusion Criteria:
  • Hematuria, glucosuria or protenuria
  • Pre-existing cardiovascular or chronic disease
  • Currently taking medications
  • Blood pressure of 136 over 86mm Hg or higher for ages 13 to 15
  • Blood pressure of 142 over 92mm Hg for those aged 16 years.
Description of Study Protocol:

Recruitment

Recruited from schools and churches, and through local recreation centers.

Design

Subjects participated in a five-day low-sodium diet immediately followed by a 10-day high-sodium diet to determine salt sensitivity status. Subjects were then randomly assigned to either a high-potassium diet or usual diet.

Intervention

High potassium diet (80 mmol per 24 hours).

Statistical Analysis

  • For electrolyte measures:
    • A series of 2X2X2 (SS vs. SR; low sodium vs. high sodium; before vs. after diet) repeated-measures ANOVAs were conducted to determine compliance with the low-sodium and high-sodium diets
    • A series of 2X2X2 (SS vs. SR; potassium vs. control group; before vs. after diet) repeated-measures ANOVAs demonstrated a significant diet-by-time interaction for UkV
  • For ABP Measures: A series of X2 analyses were performed to examine dipper and nondipper BP status as a function of SS vs. SR status and treatment groups
  • For CVR Measures: A series of 2X2X2 (SS vs. SR; potassium vs. control; before vs. after diet) repeated-measures ANOVAs were conducted for CVR change scores.

Other Findings

After treatment all of the salt-sensitive subjects who were originally classified as nondippers reverted back to a normal dipping status because of a drop in nighttime diastolic blood pressure (nighttime DBP 69±7 vs. 57±6mm Hg; daytime DBP, 69±5 vs. 67±5mm Hg). Salt-sensitive subjects in the treatment group changed from nondippers to dippers at a greater rate than salt-sensitive subjects in the control group.

 

Author Conclusion:

Nighttime BP and associated nondipping status are reversible with K+ supplementation. A positive relationship exists between dietary K+ intake and BP modulation although daytime BP and CVR responses were unchanged by the dietary intervention.
