Research Purpose:

To determine the effect of a nasogastric tube and body position as risk factors for gastroesophageal reflux.

Inclusion Criteria:
  • Orotracheal intubation > 48 hr, breathing spontaneously or mechanically ventilated
  • Tolerating continuous enteral feeds > 48 ht
Exclusion Criteria:
  • Hiatal hernia or esophagitis history
  • Parenteral nutrition
  • Shock
  • Psychomotor restlessness not controlled with sedatives
  • Use of antacids, H2 blockers, metoclopramide
Description of Study Protocol:
  • Supine or semirecumbent position by table of random numbers
  • Radioactive colloid supplied via NGT with enteral feeding held 2 hr
  • Isotope scintigraphy
Data Collection Summary:
  • Subanalysis by NGT vs no NGT
  • Mortality
  • Prevalence gastroesophageal reflux
Description of Actual Data Sample:

With NGT=50; 24 supine, 26 semirecumbent

Without NGT=20; 12 supine, 8 semirecumbent

Summary of Results:
  • Gastroesophageal reflus 81% supine vs 67% semirecumbent, p=0.26
  • Gastroesophageal reflux with NGT 74% vs 35% without, p<0.002
Author Conclusion:
The incidence of gastroesophageal reflux is high in patients with intubation NGT. Semirecumbency does not prevent gastroesophageal reflux, but it is reduced compared to supine position.
