Research Purpose:

To test a podcast designed with health behavior theories on weight loss. The hypothesis was that a weight-loss podcast designed with health behavior theories will produce a greater weight loss than a currently available podcast that is not theory-based.

Inclusion Criteria:
  • Overweight and obese men and women
  • Body mass index 25kg/m2 to 40kg/m2
  • Provided written informed consent.
Exclusion Criteria:

Unstable medical condition such as:

  • Cardiovascular disease
  • History of an eating disorder
  • Pregnancy
  • Alcohol or drug abuse
  • Tobacco use
  • Mental illness
  • Diabetes mellitus
  • Uncontrolled thyroid condition.
Description of Study Protocol:

Intervention: 

24 episodes of a currently available weight-loss podcast (control podcast) or a weight-loss podcast based on social cognitive theory (SCT) designed by the researchers (Registered Dietitians) for 12 weeks.

Statistical Analysis

  • All data collection and analyses were conducted in 2008 using intention-to treat by bringing baseline values forward for participants who attended the introductory meeting but did not complete the study
  • Between-subjects T-tests were calculated for all measures with the exception of the baseline demographic information the contained multiple categories such as education
  • For these measures a linear regression was used to predict follow-up weight adjusting for gender and group assignment and education and group assignment
  • All analyses were conducted using SPSS 16.0 for Windows software with P=0.05 used to indicate significant differences.

  • Questionnaires were completed that assessed the knowledge of weight loss topics such as how to reduce calorie intake and increase exercise, food intake (Prime Screen Questionnaire), physical activity (short International Physical Activity Questionnaire IPAQ) and elaboration (the elaboration likelihood model), as well as questionnaires that assessed user control, cognitive load and social cognitive theory constructs (self-efficacy, expectancies and outcome expectations)
  • Changes in fruit, vegetable and fatty food intake were assessed using the Prime Screen Questionnaire by averaging the consumption category reported for servings of all vegetables, fruits and all fatty foods
  • Changes in physical activity were measured by self-report and not by an objective method such as the use of a pedometer. Using a Likert scale, participants were asked pre- and post-intervention to rate their knowledge of six weight-loss related topics such as dining out, exercise and calorie restriction.

Description of Actual Data Sample:
  • Initial N: 120 men and women assessed for eligibility, 94 were randomized
  • Attrition (final N): 78 used for analysis (41 intervention group, 37 control)
  • Age: Mean age in control group (39.6±12.2 years), mean age in enhanced group (37.7±11.8 years)
  • Ethnicity:
    • Control group: 17% black, 78% white and 5% other
    • Enhanced group: 13% black, 85% white and 2% other
  • Other relevant demographics:
    • Control group: 31% high school or partial education, 50% college, 19% graduate degree
    • Enhanced group: 20% high school or partial education, 37% college, 43% graduate degree
  • Anthropometrics: There were no significant differences in baseline characteristics between completers and non-completers. The control and enhanced group did not differ significantly for any of the variables except that there were a disproportionately greater number of subjects in the control group without a graduate degree.
  • Location: North Carolina.
Summary of Results:

Key Findings

Enhanced group participants (N=41) had a greater decrease in weight (-2.9kg±3.5kg enhanced group vs. -0.3±2.1 control group; P<0.001 between groups) and BMI (-1.0±1.2kg/m2 enhanced group vs. -0.1±0.7kg/m2 control group; P<0.001 between groups) than the control group (N=37) and had greater weight loss related knowledge (P<0.05), elaboration (P<0.001) and user control (P<0.001) and less cognitive load (P<0.001).

 

 

 

Author Conclusion:

The results of this study suggest that the use of behavioral, theory-based podcasting may be an effective way to promote weight loss.

Limitations to the present study are a mostly white and highly educated sample. Dietary data were measured using a brief questionnaire instead of more detailed methods such as a 24-hour recall or food record. Physical activity was also measured using a questionnaire when less subjective methods such as a pedometer may have provided more accurate results.
